Skilled Nursing
Our skilled nursing services provide 24-hour supervision and sophisticated medical services for residents who have more complex medical needs or health challenges.
Our resident-oriented care at Brookfield Rehabilitation & Speciality Care means that we get to know each resident individually to provide the best possible care and services for his or her individual needs. We also aim to provide a home-like atmosphere so our residents can enjoy peace of mind and comfort at our community.
Short-Term Rehab
Our on-site rehabilitation services are for those who need rehabilitation, per a physician's orders, after a hospital stay, an injury, an illness, or surgery. We offer physical therapy, occupational therapy, speech language pathology and more to meet the specific needs of each individual.
Short-Term / Respite Stays
Brookfield Rehabilitation & Speciality Care also has accommodations for short-term stays for seniors who need care after a hospital stay, or when their family members / caregivers travel out of town or simply need a short break from the stresses of caregiving. Short-term stays can vary from days to months.
Alzheimer's/Memory Care
At Brookfield Rehabilitation & Speciality Care, our unique approach to caring for residents with Alzheimer’s or memory loss includes helping each person to engage in the world around them and to reach their highest potential at each moment.
Hospice
We also offer hospice care so that our residents who are near the end of life can remain comfortably in the same place while both they and their family receive comfort and care during this difficult time.
Discharge / Returning-to-Home Planning
Our personalized discharge assessment and planning means that we carefully coordinate with our residents and their home caregivers to make sure that all needs will be met after leaving Brookfield. We carefully assess our residents' status and anticipated future needs and plan their care in collaboration with medical providers, caregivers and the resident themself.
