Along with an on-site dialysis center, we offer physical therapy, occupational therapy, infusion therapy and much more in a warm and caring environment. We also provide recreational activities to encourage our residents to socialize and keep active.

A New Approach to Rehab to Home Care

You’ve been in an auto accident, had joint replacement surgery, or suffered a stroke and you’re released from the hospital -- but not to go home! Where will you or your loved one go if you need to be admitted to a rehabilitation center before you can go home?
It was an eye-opening ribbon cutting ceremony recently at Brookfield Rehabilitation & Specialty Care (BRSC) – the ribbon cutting was to celebrate the completion of Phase I of their remodeling project and to introduce their new Rehab to Home Program. This is a program of BRSC corporate office – Five Star Quality Care. BRSC has jumped at the opportunity to embrace this new program and the services it will bring to the community and their patients.
The Rehab to Home Program is about patient moderated care where the patient sets the pace for their care. This program, and the design of the facility, is a wonderful alternative to care for active adults after a fall, auto accident, joint replacement surgery, etc.
Speaking from personal experience, when my mother needed in facility rehab after a hip replacement surgery, she was in a nursing home with folks that were, well, those that needed 24/7 skilled nursing care. For anyone who has been in that environment, you know it is not one that is motivating or uplifting. With the remodel at BRSC, patients that are there for the Rehab to Home Program have their own wing, their own dining area, access to therapy seven days a week (so regardless when you arrive, you will get rehab) and a personalized therapy and hospitality team to assist them in their recovery.
Through this program, BRSC specializes in orthopedic rehab, cardiac and pulmonary care, stroke recovery and neurological conditions. And when it comes to therapy, BRSC has state of the art therapy equipment – ranging from Biodex Balance Trainer, Pro-Gym Cross Trainer, Diathermy, ultra sound & electrical stimulation. In addition to all the equipment, there is the standard physical therapy, occupational therapy and speech therapy - and an added plus, the therapy room is bright and spacious. Phase II of the remodel, with completion expected in 2012, will provide 16 private suites for the patients in the Rehab to Home Program.
In addition to this new program, BRSC continues to offer patient centered care for those requiring 24/7 skilled nursing care.
